1. Serious Leg Fracture

Injury:

This gentleman sustained a serious lower leg fracture following a work related accident. The fracture was treated with internal pinning and plating via surgery.

Rehabilitation:

The file was referred to AIG Medical Management Services (Ireland) three weeks post accident. MMS contacted the gentleman to introduce MMS. He declined the offer of case management.  

Two months later, a solicitor’s letter was received by the claims department. The case was re-referred to MMS and we subsequently wrote to the gentleman’s solicitor requesting permission to case manage. The gentleman’s signed consent was received one month later. At that stage, the gentleman had been discharged from primary care and hospital based physiotherapy but reported that he was having a lot of difficulty with pain/soreness around his foot.

AIG Medical Management Services (Ireland) immediately organised private physiotherapy and also facilitated payment of support orthotic insoles. The case manager negotiated a return to work via a phased basis and the gentleman returned to full duties at work some 5 months post accident

2. Intermittent Absence due to Depression and Alcoholism

Injury:

This gentleman was referred to AIG Medical Management Services (Ireland) for case management as he had been out of work intermittently due to alcohol addiction and had received treatment for this. However, he subsequently was diagnosed with depression and was again frequently absent from work. His employer wished to support him with these issues and referred him to MMS for further assistance.

Rehabilitation:

A file was opened for this gentleman and the Case Manger contacted him on the same day. His consent form was received four days later. The Case Manager proceeded to arrange counselling therapy with a specialist in Alcohol and Addiction Counselling and updated his GP of same. 

The Case Manager liaised regularly with him and his counsellor and both parties reported that progress was continuously being made. He completed 4 sessions of counselling therapy in all and thereafter attended a group meeting for addiction support. This gentleman remained in work throughout his counselling therapy. 

This file was referred for closure as he reported that he had received substantial assistance on how to deal with events and no further treatment recommendations were suggested.

The company reported that the employee’s level of absence had decreased substantially and he reported that he was better able to concentrate with activities in work and deal with matters as they arise. The employer was perceived as being supportive, while protecting their employee’s confidentiality. 

For this gentleman, he now has an established network of relevant service providers should he require further treatment in the future.

3. Back Injury At Work - Requiring Vocational Consultancy

Injury:

This 32 year old operations clerk in a large freight company, was involved in an accident during working hours and sustained a lower back injury.

Rehabilitation:

The file was referred to AIG Medical Management Services (Ireland) two weeks post accident. The Case Manager liaised with this gentleman’s medical practitioners and organised physiotherapy at his GP’s request. Initial reports received from his physiotherapist indicated a more sinister diagnosis and therefore an early neurosurgical consultation was organised.

Following intensive investigation including early MMS I scanning, this gentleman underwent surgery for removal of a central disc protrusion three weeks post referral to MMS. Post operative physiotherapy and rehabilitation was organised by the Case Manager as directed by the treating practitioners.

During early post operative rehabilitation MMS sought a physical demands analysis of this individual’s job and in consultation with his doctors, recommended vocational consultancy as it was obvious that he would be unable to return to his pre-injury job which involved a high degree of pulling, pushing and lifting.

AIG Medical Management Services (Ireland) Vocational consultant carried out a thorough skills analysis on this man and was able to match his profile with an alternate job within the company, once training had been provided. This employee returned to work to his new position as Customer Services representative having completed rehabilitation and re-training, thirteen months post date of accident.

This individual was unable to return to his original job and may potentially have never returned to work.

4. Injuries Sustained In A Road Traffic Accident

Injury:

This 41 year old lady complained of headaches and soft tissue pain in her neck, shoulders and back following a road traffic accident on As a result of her injuries, she was unable to remain in work. The file was referred to MMS by the Insurer 5 days following her accident.

Rehabilitation:

The case manager contacted this lady on the day of referral, received her consent within 4 days and immediately arranged 6 sessions of private physiotherapy as per the GP’s recommendations.

Her initial progress was slow and she continued to report of severe headaches. She was reviewed by her GP who recommended a cervical MRI scan and advised her to stop attending physiotherapy until the results were obtained. The Case Manager arranged an immediate private scan and forwarded a copy of the report to her GP. On review of this lady, she was recommended to recommence physiotherapy treatment for a further 6 sessions. This was then arranged by MMS.

She progressed well with the second course of physiotherapy and reported that her headaches and injuries were resolving. She returned to work to her pre-injury job three months post accident.

She continued with her physiotherapy along with her recommended swimming exercises. She completed a further 4 sessions of physio as recommended by her physiotherapist and she received full medical sign-off. The file was closed as she had remained in full time work and no further treatment had been recommended.

Thanks to the intervention of AIG Medical Management Services (Ireland) this lady was able to avail of immediate physiotherapy treatment. By having direct communication with her medical treating practitioners, MMS could assess the appropriate treatment required and co-ordinate it. She successfully returned to work 3 months following her accident.

The Insurer commented that the intervention of MMS was very successful in this case. The claim was settled prior to The Injuries Board's assessment of the case. Therefore, no legal costs were incurred.

This case highlights how MMS’s involvement can benefit both the injured party and the employer and insurers alike.
